I didn't put one within for mine and after 1 day my phone go into immediate lockdown because I couldn't find out the 20 digit code...
I took it wager on to the AT&T store and they had no theory what happened, so they give me a new sim card =]

Every company is doing that presently mighty annoying. It may be the last four digits of you SSN or phone number. Best bet is to phone up the company.
